We are seeking a Licensed Practical Nurse (LPN) to join our team at White Oak Health Campus in Monticello, Indiana.
About the Role
This is an exciting opportunity for an LPN to work in a dynamic and supportive environment where you will have the chance to make a real difference in the lives of our residents. As an LPN at Trilogy, you will be responsible for providing high-quality patient care, working closely with our Registered Nurses to assess patients and administer medications, and educating patients and their families about disease treatment plans.
Responsibilities
- Contribute to the assessment of patients in close collaboration with Registered Nurses
- Administer medications and observe patients to ensure they do not have adverse reactions
- Perform routine lab tests and therapeutic services
- Educate patients and their families about recommended disease treatment plans
- Identify safety hazards and emergency situations to keep residents safe

Requirements
- BLS Provider obtained within 1 month of hire date or job transfer date
- Licensed Practical/Vocational Nurse obtained prior to hire date or job transfer date
- High school diploma or GED
