What are the responsibilities and job description for the Clinical Liaison RN / LPN position at Whitefish Care and Rehabilitation?
Whitefish Care and Rehabilitation -
We are looking for a self starter with a positive attitude and the ability to establish relationships with the community.
MUST BE A LICENCED NURSE
- Increase census through cultivating relationships with multiple referral sources including hospitals, LTACs, ALs, ILs, and the community at large
- Assess patients and work to build relationships with area hospital case managers, social workers and physicians
- Attend hospital meetings, coalition meetings, and networking meetings to grow brand awareness
- Reflect the quality of care and services provided, including on-site short-term, respite, and long-term care in our skilled nursing facility
- Act as a self-starter, skilled communicator and positive motivator
- Able to organize and prioritize many tasks effectively
- Ability to utilize both local and company resources in the execution of job responsibilities
- Must have the ability to network at all levels with strong drive, a passion for success, and a proven track record of census growth and development
