We are looking for a kind and compassionate assistant for our Social Services department. The qualified individule should be able to maintain a professional relationship with the residents and staff of our long term care facility.
Job Duties include assessments, admission meetings, discharge planning, MDS's and care plans, assistance with housing and community resources.
Experiance preferred but not required. This is a full time positon with flexible days.
