What are the responsibilities and job description for the Support Coordinator/ Patient Advocate position at Whitemarsh House?
General Description:
The Supports Coordinator works with the management team, clients, families and or guardian’s and government agencies to assure the resident get the health benefits and services needed.
Reports to: Executive Director and/or Director of Nursing.
Qualifications:
College degree or High School diploma with 2 years’ experience or with proven program development and supervisory experience. This individual must be an organized person with good computer knowledge. His/her specialty must include Direct Care, TBI care and/or psychiatric care. Strong communication, organizational, and problem-solving skills.
Specific duties but not limited to:
- Coordinate various services, including medical care, therapy, personal care, and activities, ensuring seamless transitions and optimal outcomes
- Advocate for residents' rights and needs, providing emotional support and guidance to residents and their families
- Serve as a central point of communication, connecting residents, families, and management to facilitate effective care
- Help residents and families navigate complex systems, such as Medicaid, Medicare, SSDI and other community resources, to access necessary benefits and support
- Maintain accurate and up-to-date records of residents' care plans, progress, interactions, and insurance, waivers and billing sheets.
- Linking individuals and/or children and their families to community resources and supporting them in identifying and accessing formal and informal resources independently
- Preparing reports on client status, progress, or other issues as needed
- Marketing for potential admissions, becoming the liaison with insurance companies and community outreach, establishing relationships with doctors, nurses and therapists to gain referrals. Check all messages on voicemail and website return calls and emails.
- Completing documentation and other paperwork in a timely and professional manner including but not limited to preadmission paperwork, authorizations, discharge paperwork, billing sheets etc.
- Collaborate with the corporate office and or billing company to ensure accuracy and timely authorizations are processed correctly without any interruption of benefits
**The rate of pay is $21 per hour, with the potential for a salary increase following a 90-day evaluation period.
