GENERAL SUMMARY:

The Resident Care Coordinator ensures each resident is provided and receives the necessary care and services to attain or maintain the highest practical physical, mental and psychosocial well-being consistent with the residents’ comprehensive assessment and plan of care.

PRINCIPLE DUTIES:

Essential Job Duties:

  • Asses, evaluate, plan and implement resident care plans and respond to resident needs.
  • Ensure and oversee adequate staffing including meeting posting requirements.
  • Makes daily rounds to assure that the department personnel are performing required duties, that practices are in compliance with applicable regulations and that appropriate resident care is being rendered.
  • Ensures that all physician orders are implemented.
  • Ensure all residents are free from abuse, neglect, misappropriation of resident property, and exploitation.
  • Keeps physician and family aware of change in resident’s condition including diagnostic tests.
  • Must be available to report to work in the event of an emergency.
  • Provide maximum resident-care services within limits defined by delegated tasks.
  • Plans, organizes, develops and directs the overall operation of the Reflections and Assisted Living Neighborhood in accordance with federal, state, and local laws and WhiteStone regulations and guidelines.
  • Completes all WhiteStone Assisted Living training and state mandated training or required ongoing training per state guidelines.
  • Demonstrates competence and leads programming with the assistance of the Life Enrichment Coordinator to ensure that programming is scheduled and conducted in a way that reflects the resident’s capabilities and interests.
  • Designs, schedules and facilitates the Heartfelt Connections program ensuring a variety of dementia appropriate activities.
  • Facilitates and monitors the Negotiated Service Plans to ensure completion of tasks including activities of daily living and engagement in daily programming.
  • Partners with AL Manager by alerting to any change of condition of the resident including tracking incident reporting
  • Participates in Move In process by attending initial assessment with AL Manager and communicating resident needs and preferences to team member
  • Coordinates bi annual Negotiated Service Plan Meetings with the resident and family.
  • Creates and monitors the Daily Assignment Sheets.
  • Utilizes the assessment process and findings to determine the care levels and appropriate variable staffing.
  • Partners with the Executive Chef to ensure resident’s needs and preferences are met. * Participates in the dining experience daily.
  • Uses Best Practices to ensure programming meets WhiteStones standard benchmarks
  • Participate in grievance investigations.
  • Other job duties as assigned within scope of practice.

Other Duties:

  • Assists in admission and discharge procedures as per North Carolina Adult Care and facilitates warnings and corrective action under the supervision of the Director of Nursing.
  • Responds to resident emergency call system as indicated.
  • Makes rounds with physicians and transcribes orders.
  • Participate in committees as assigned.
  • Maintains the self‑respect, personal dignity, and physical safety of each patient.
  • Assures continuity of services and care.
  • Attends in-service training and education sessions, as assigned.
  • Performs specific work duties and responsibilities assigned by supervisor.

QUALIFICATIONS:

  • RN/LPN currently licensed by the State.
  • At least 2 years nursing experience. Geriatric involvement required.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

Frequent standing, stooping, bending, stretching, squatting; must be able to stand/walk for up to 70% of the work day; must be able to lift , transfer, turn and position residents weighing up to 125 lbs. unassisted, or with the assistance of one for a resident weighing over 125 lbs. May be subjected to offensive odors and combative behavior. Must be able to lift and carry up to 20 lbs. frequently and 50 lbs. occasionally. May be exposed to communicable diseases including the HIV and HBV viruses. Must be able to work paying close attention to detail with frequent interruptions.
