Whitney Nursing and Rehabilitation is seeking a dedicated MDS Coordinator to join our skilled nursing team. Located in the heart of Whitney, Texas, our facility is committed to providing exceptional care in a compassionate and supportive environment.
As MDS Coordinator, you’ll play a crucial role in ensuring the accuracy and efficiency of our resident assessments and care plans. If you’re a licensed RN or LVN with strong clinical judgment, outstanding organizational skills, and a drive to make a difference.
Schedule:
- Monday through Friday
- 8:00 AM to 5:00 PM
Responsibilities:
- Coordinate and complete timely and accurate MDS assessments and care plans in accordance with state and federal regulations.
- Collaborate with the interdisciplinary team to develop, implement, and monitor resident-centered care plans.
- Ensure accurate documentation that reflects the residents' clinical status.
- Track and analyze quality measures, QMs/QRP, and PDPM reimbursement metrics.
- Participate in care plan meetings and communicate with residents and families as needed.
- Serve as a resource for nursing staff regarding documentation best practices and assessment processes.
- Utilize PointClickCare for MDS documentation and care planning.
Qualifications:
- Current Texas RN or LVN license in good standing (Required)
- 1–2 years of experience as a nurse in long-term care preferred, but not required
- Previous experience as an MDS Coordinator or in a similar role (Preferred)
- Knowledge of PDPM, QAPI, and Medicare/Medicaid regulations
- Proficiency with MDS 3.0, electronic health records, and care planning
- Familiarity with PointClickCare platform is a plus
- Strong communication, organizational, and leadership skills
