Whitney Nursing & Rehab is seeking both Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) to join our clinical team. Responsibilities:
- Perform comprehensive nursing assessments and ongoing evaluations to guide resident care plans.
- Administer medications, wound care, IV therapy, and treatments in accordance with physician orders and nursing protocols.
- Monitor for clinical changes and intervene promptly to prevent complications and optimize resident outcomes.
- Collaborate with interdisciplinary teams including physicians, rehab specialists, social workers, and CNAs to implement holistic care.
- Document all assessments, care interventions, and outcomes with accuracy in our EMR system.
- Educate residents and family members about chronic condition management, discharge planning, and medication adherence.
- Participate in quality assurance programs and continuous improvement initiatives.
Qualifications:
- Active RN or LVN license in the state of Texas
- Current BLS certification (preferred)
- Previous experience in long-term care, acute care, or rehab setting is preferred but not required
- Proficiency with clinical documentation and EMR systems
- Strong clinical judgment, problem-solving abilities, and commitment to patient advocacy
