Your Role:
As a Licensed Social Worker at Whitney, you'll play a vital role in ensuring the emotional and social well-being of our residents. You will collaborate with an interdisciplinary team to provide holistic care, facilitate smooth transitions between care settings, and advocate for the needs of our residents and their families.
Responsibilities:
- Conduct comprehensive assessments to identify the needs and concerns of residents and their families.
- Develop and implement individualized care plans that address the psychosocial needs of residents.
- Provide counseling and support to residents and families navigating the challenges of illness, recovery, and aging.
- Connect residents and families with community resources and support services.
- Collaborate effectively with the interdisciplinary team, including nursing, dietary, administration, and activities staff.
- Maintain accurate documentation and ensure compliance with regulatory requirements.
Qualifications:
- Licensed Social Worker (LSW, LMSW, LBSW, LCSW) in good standing with the Texas Board of Social Work Examiners.
- Licensed by and in good standing with the Texas Board of Social Work Examiners.
- Experience in a medical social work setting is highly preferred.
- A compassionate and empathetic approach to resident care.
- Excellent communication, interpersonal, and problem-solving skills.
