Substance Use Disorder Counselor - Community Prevention Treatment Services (Req 100859)Albany, NY ( DeWitt Street Albany NY USA 12207)ApplyDescriptionGENERAL RESPONSIBILITIES : The Substance Use Disorder (SUD) Counselor provides prevention, harm reduction and recovery support to persons with substance use, mental health disorders and chronic co-morbid health issues. This position plays an important role in integrating services to successfully ensure that individuals across the health-care domains receive all services necessary to experience optimal physical and mental health and recovery. The SUD Counselor conducts screenings and assessments that address integrated health care needs. Coordinates and consults with other staff and service providers to support the attainment of service plan goals and positive health outcomes.SPECIFIC RESPONSIBILITIES :

• Provide culturally informed, evidence-based, co-occurring support services including screening / assessment, outpatient services and Medication Assisted Treatment for individuals at risk of HIV.
• Maintain knowledge and skill of substance use disorders, harm reduction and evidence based treatment approaches including but not limited to Motivational Interviewing.
• Maintain a caseload of patients, as assigned.
• Implement brief intervention and referral to treatment (SBIRT) services to detect substance abuse, mental health and co-occurring conditions and refer to on-site and local services for appropriate treatment.
• Utilize screening tools such as but not limited to PHQ-9, GAD-7, SDOH and provide linkage to services.
• Work in conjunction with the multidisciplinary treatment team.
• Provides individual and group psycho-education, coordination of care and counseling.
• Maintains and protects accurate and timely documentation in case records according to the agency and governing bodies.
• Assists in providing a safe and respectful environment for treatment.
• Attends and participates in all assigned meetings and committees.
• Develops and networks with other community service providers to provide the full continuum of care needs identified by the multidisciplinary treatment team and supervisor.
• Makes and completes all referrals to other community service providers as appropriate.
• Continues to seek and develop professional growth and activities.
• Maintains a professional relationship with patients, peers, other providers and the community.
• Refers for preventive testing and treatment for HIV, HCV and STIs.
• Encourages and provides introduction to self -help programs and any other community resources appropriate to patients' needs.
• Demonstrates excellence in both internal and external customer service.
• Understands and is able to effectively communicate HIPAA compliance, corporate compliance and client confidentiality.
• Ensures and / or remains in compliance with local, state, and federal regulation, i.e. DHHS HRSA, SAMSHA, NYSDOH, and all accreditation standards (e.g. Joint Commission and NCQA- PCMH).
• Adheres to the National Patient Safety Goals as defined by the Joint Commission and WhitneyM. Young Jr. Health Center.
• Completes other duties as assigned.RequirementsMINIMUM QUALIFICATIONS : Bachelor’s Degree in social work, human services, or a healthcare related field; one year of experience working with individuals with substance use and mental health disorders. Ability to maintain strict confidentiality with good grasp of ethical codes / conduct. Must have strong verbal and written communication skills.PREFERRED QUALIFICATIONS : Master’s Degree in social work, human services or a healthcare related field with up to one year of experience working with individuals with substance use and / or mental health disorders.All qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability, status as a protected veteran, or any other legally protected status.Salary Range : $23.00 - $25.50 hourly
