Certification Details

BLS - Basic Life Support
ACLS - Advanced Cardiac Life Support

Job Details

Must have MA license in hand for acceptance

1-year plus experience in Case Management, LTAC / Rehab

CM experience must be specific for hospital discharge planning (rehabilitation hospital experience a plus but not required)

Familiar with EMR and / or documentation expectations for role (ACMA SOP)

Familiar with Continuity of Care / Transition of Care requirements (CMS standards)

Job Requirements

MA license required

1 years of Case Management experience

Experience in hospital discharge planning preferred

Familiarity with EMR and documentation expectations

Knowledge of Continuity of Care / Transition of Care requirements

Schedule Information

Shift : Mon-Friday 8-5p

Shift is 5x8 days but any variation to 8 hour shift (staying late / coming in early) has to be discussed and approved by manager

Unit Specific Information

Unit specializes in rehabilitation and case management

Additional Details

Government issued photo ID required

MA CORI Statewide Release – All travelers must complete ASAP

TB Questionnaire required for all travelers
