SUMMARY OF POSITION:

The Recovery Coach reports to the Associate Director of Behavioral Health and will have an active role in the care coordination for patients with a substance use disorder or a history of overdose. The Recovery Coach will provide recovery education to patients for every phase of the recovery journey from pre-recovery engagement, recovery initiation, recovery stabilization, and sustained recovery maintenance. The Recovery Coach will serve as an integral member of patients’ treatment team, with frequent collaboration with other internal and external providers. The Recovery Coach will also participate in case management of participants enrolled in the Moms Do Care Program and will be included in all required grant activities. The Recovery Coach will participate in data collection responsibilities, facilitate workshops and data tracking systems (Virtual Gateway and GPRA) and will attend trainings and meetings related to the Moms Do Care Program.

ESSENTIAL FUNCTIONS:

  • Serve as a role model for both individuals in recovery and staff by demonstrating that recovery is possible.
  • Assist and coach individuals in recovery to identify their personal interests, goals, strengths, and areas for growth.
  • Recovery Planning: Facilitate the transition from a professionally directed service plan to a self-directed recovery plan through personal coaching. The goal is to shift from professionally assisted recovery initiation to personally directed, community-supported recovery maintenance.
  • Promote self-advocacy by ensuring that individuals in recovery have their voices heard and that their needs, goals, and objectives remain the focal point of rehabilitation and clinical services.
  • Actively identify and support connections to community resources, including recovery groups, educational and vocational programs, social and cultural activities, spiritual resources, mutual self-help groups, and professional services that align with the individual’s goals and interests. This effort will involve collaboration among the individual, agency staff, and other relevant stakeholders.
  • Support engagement with community-based mutual self-help groups and connect individuals to appropriate professional resources when necessary. Provide encouragement and hope by promoting opportunities for meaningful involvement in work, education, relationships, physical activity, and self-directed hobbies.
  • Maintain contact with individuals after they leave the program, providing scheduled follow-ups to ensure ongoing success. Offer re-engagement support in collaboration with agency staff as needed, emphasizing long-term engagement, encouragement, and support.
  • Recruit, engage, and enroll eligible pregnant and parenting women into the Moms Do Care program.
  • Oversee tracking and data entry for the Moms Do Care program.
  • Attend weekly supervision meetings with the Associate Director of Behavioral Health.
  • Perform other duties as assigned by the Director and/or Associate Director of Behavioral Health.

QUALIFICATIONS & REQUIREMENTS:

  • High school diploma or equivalent; completion of Recovery Coach training preferred
  • Lived experience with a minimum of five years of sustained recovery
  • Strong knowledge of the targeted communities, their needs, and effective outreach strategies
  • Excellent written and verbal communication skills
  • Ability to demonstrate linguistic and cultural competency
  • Proficiency in Microsoft Office

Job Type: Full-time

Pay: $28.00 - $30.00 per hour

Expected hours: 40 per week
