Position Overview:
We are seeking an independent, compassionate, and reliable Care Manager to join our team in the Groton, Nashua, and Chelmsford areas of Massachusetts and New Hampshire. This full-time position offers the opportunity to make a meaningful impact while leading caregiver teams, overseeing client care, and ensuring our high standards of service are upheld.
This role requires flexibility, including weekend shifts, overnight shifts, and participation in an on-call rotation with stipends for on-call availability. Candidates must have 4 years of experience in in-home senior care and a proven ability to manage care plans, communicate effectively, and provide compassionate support.
Key Responsibilities:
Manage and monitor the care provided to clients, ensuring tasks are completed according to care plans and high-quality service is delivered.
Supervise caregiver teams, provide guidance, and resolve issues as they arise. Train and mentor staff to maintain excellent standards of care.
Create and adjust caregiver schedules to ensure full coverage, with flexibility to meet clients’ needs.
Build strong relationships with clients and their families, serving as the main point of contact for care updates and addressing concerns.
Collaborate with clients, families, and caregivers to develop and update individualized care plans based on evolving needs.
Conduct regular in-home visits to ensure tasks are completed, client satisfaction, and adherence to care plans.
Maintain accurate and timely client records, caregiver notes, and care reports.
Be available for overnight, weekend, and on-call shifts, ensuring care needs are met around the clock.
Qualifications:

4 years in in-home senior care, with demonstrated knowledge of caregiver best practices.

Independent, strong communicator, and flexible with schedules, including weekends and overnights.

A genuine passion for providing exceptional care to the elderly and supporting their well-being.

Must have your own car and valid driver’s license (mileage reimbursement provided for work-related travel).

Ability to adapt to varying schedules and take on-call responsibilities as needed.
