JOB SUMMARY
The Case Manager is responsible for assessing the strengths and needs of clients, linking clients with appropriate resources, creating housing and service plans, monitoring clients’ progress towards goals established in service plans and empowering clients towards self-sufficiency. Case Manager is responsible for practicing active listening, providing trauma-informed care, and acknowledging and documenting client progress.
KEY RESPONSIBILITIES
- Develop and oversee comprehensive care plans for patients, addressing their medical, mental health, and social needs.
- Coordinate with healthcare providers, social services, and community resources to ensure seamless care transitions and continuous support for patients.
- Conduct regular assessments of patient progress and adjust care plans as needed.
- Monitor patient care outcomes and implement quality improvement initiatives to enhance service delivery.
- Foster strong relationships with healthcare providers, community organizations, and other stakeholders to enhance collaborative care efforts.
- Act as a liaison between patients, families, and healthcare teams to facilitate effective communication and patient advocacy.
- Participate in multidisciplinary team meetings and case conferences to discuss patient care and coordinate services.
- Identify opportunities for program development and implement innovative care management strategies to improve patient outcomes.
- Other duties as assigned by the supervisor.
POSITION REQUIREMENTS & QUALIFICATIONS
- Basic Life Support (BLS) within 2 weeks of start date
- Experience working with homeless populations and supportive housing
- Experience in case management, mental health, and other supportive services
- Excellent verbal communication, interpersonal, and organizational skills for coordinating care between patients and their care team.
- Strong leadership and team management skills needed for managing a team of care managers, leading meetings, and training new care managers.
- Proficient in care coordination and electronic health record systems.
- Ability to work independently and make decisions in a fast-paced environment.
