Key Responsibilities :
Leadership and Strategy :
- Provide clinical leadership and strategic direction for programs serving unhoused populations, including recuperative care, street medicine, and urgent care.
- Collaborate with executive leadership to develop and implement initiatives addressing the unique healthcare needs of individuals experiencing homelessness.
- Promote a mission-driven culture of dignity, equity, and respect in all aspects of care delivery.
Program Oversight : Recuperative Care :
Oversee clinical services for recuperative care programs, ensuring patients discharged from hospitals receive appropriate follow-up care, support, and housing connections.Collaborate with multidisciplinary teams, including case managers, housing navigators and mental health specialists, to address medical and psychosocial needs.Develop protocols to manage common post-hospitalization conditions and coordinate care transitions effectively.Street Medicine :
Lead clinical efforts in delivering healthcare directly to individuals in encampments, shelters, and on the streets.Develop and standardize mobile healthcare protocols, emphasizing harm reduction, chronic disease management, and preventive care.Partner with outreach teams to build trust and address barriers to accessing care, including mental health and substance use support.Urgent Care :
Ensure the delivery of high-quality, compassionate care in the urgent care setting, addressing acute illnesses and injuries common in homeless populations.Optimize workflows to reduce wait times and improve access for patients with limited resources.Incorporate culturally competent, trauma-informed care practices into urgent care operations.Clinical Responsibilities (30% of Time)
Provide direct patient care across programs, modeling excellence in clinical skills, cultural competence, and patient-centered care.Diagnose, treat, and manage a wide range of acute and chronic medical conditions, with sensitivity to the challenges faced by homeless individuals.Supervise and mentor clinical staff, fostering professional development and continuous learning.Participate in street medicine rounds, urgent care shifts, and onsite recuperative care visits to ensure alignment with organizational goals.Administrative Responsibilities (70% of Time)
Quality Assurance and Program Development :
Develop and implement clinical guidelines, policy and procedures, protocols, and workflows tailored to the needs of unhoused populations.Monitor clinical performance metrics and outcomes, using data to drive quality improvement initiatives.Ensure compliance with regulatory standards and best practices in all care settings.Staff Recruitment and Training :
Recruit, train, and supervise a multidisciplinary team, including nurse practitioners, physician assistants, and allied health professionals.Provide ongoing training on trauma-informed care, cultural humility, and strategies for addressing social determinants of health.Conduct regular performance evaluations and foster a culture of collaboration and accountability.Community Engagement and Advocacy :
Build and maintain partnerships with hospitals, shelters, community organizations, and government agencies to enhance care coordination.Advocate for policies and funding that address health disparities, homelessness, and access to healthcare.Represent the organization at community forums, conferences, and stakeholder meetings.Risk Management and Compliance :
Ensure adherence to safety protocols, infection control practices, and ethical standards.Oversee documentation, billing, and credentialing processes to meet legal and financial requirements.Lead risk management initiatives, including incident reviews and audits.Additional Responsibilities
Serve as a clinical resource for staff and community partners, addressing complex cases and offering guidance on care strategies.Contribute to grant writing and reporting by providing clinical expertise and data on program impact.Stay informed about emerging trends and best practices in homeless healthcare, integrating innovative approaches into programs.Qualifications :
Education and Certification :
NP or PA degree from an accredited institution.Board certification in a relevant specialty.Active and unrestricted medical license in the state of practice.DEA LicenseBLS CertificateExperience :
Minimum 5 years of clinical experience, with at least 3 years in a leadership or administrative role.Experience in nonprofit, community health, or public health settings is strongly preferred.Demonstrated ability to balance administrative duties with direct patient care effectively.Skills and Competencies :
Passion for serving underserved and vulnerable populations.Strong leadership, organizational, and interpersonal skills.Expertise in trauma-informed care, cultural competence, and health equity.Knowledge of quality improvement processes, regulatory compliance, and performance metrics.Excellent clinical skills and the ability to mentor and inspire a multidisciplinary team.Able to learn and train others on electronic health record documentation.Work Environment
Split between administrative office responsibilities and direct clinical care in community health or similar settings.Occasional travel to external sites, partner organizations, and community events.
