The Enhanced Care Manager is a pivotal role responsible for overseeing the delivery of comprehensive care management services to patients, ensuring high-quality care, and acting as a primary provider of Enhanced Care Management (ECM). This role involves coordinating care across multiple disciplines, developing care plans, and working closely with patients and their families to achieve optimal health outcomes.
Key Responsibilities:
- Responsible for coordinating with individuals and/or entities to ensure a seamless experience for patients and non-duplication of services
- Outreach to patients for ECM services and provide care coordination whenever necessary. Offer services where patient lives, seeks care, or finds most easily accessible and within health care plan guidelines as applicable.
- Connect patients to social services and support services that patient may need, including transportation.
- Develop and oversee comprehensive care plans for patients, addressing their medical, mental health, and social needs.
- Coordinate and advocate on behalf of patients with healthcare providers, social services, and community resources to ensure seamless care transitions and continuous support for patients.
- Conduct regular assessments of patient progress and adjust care plans as needed.
- Monitor patient care outcomes and implement quality improvement initiatives to enhance service delivery.
- Accompany patient to office visits as needed and according to health care plan guidelines.
- Monitor patient treatment adherence (including medication)
- Use motivational interviewing, trauma-informed care, and harm-reduction approaches. Provide health promotion and self-management training.
- Foster strong relationships with healthcare providers, community organizations, and other stakeholders to enhance collaborative care efforts.
- Act as a liaison between patients, families, and healthcare teams to facilitate effective communication and patient advocacy.
- Participate in multidisciplinary team meetings and case conferences to discuss patient care and coordinate services.
- Identify opportunities for program development and implement innovative care management strategies to improve patient outcomes.
Requirements
- Medical background, knowledgeable in medical terminology and client care with appropriate training and oversight
- Excellent verbal and interpersonal communication, as well as organizational skills for coordinating care between patients and their care team.
- Proficient in care coordination and electronic health record systems.
- Ability to work independently and make decisions in a fast-paced environment.
