JOB SUMMARY
The Medical Coordinator of Recuperative Care will oversee and coordinate medical services provided to clients in a recuperative care setting for individuals experiencing homelessness. This role ensures that clients receive comprehensive, high-quality care tailored to their unique medical and social needs. The Medical Coordinator/Manager serves as a key liaison between healthcare providers, case management teams, and community resources, fostering a holistic approach to recovery and wellness.
KEY RESPONSIBILITIES
Medical Oversight and Coordination
Facilitate medical intake for newly admitted clients in collaboration with referral coordinator.
Schedule and coordinate medical appointments, transportation, and follow-up care for clients.
Ensure proper documentation of medical services, including health histories, treatment plans, and medication reconciliation in EMR.
Supervise MA with daily medication administration, provide on the spot education to optimize clients compliance. Facility daily MAR entry.
Monitor clients' progress and address any barriers to achieving care goals.
Client Advocacy and Support
Serve as a point of contact for clients regarding their medical concerns and needs.
Educate clients on managing chronic conditions, medication adherence, and preventive care.
Facilitate communication between clients and healthcare providers to enhance understanding and engagement.
Collaboration and Communication
Work closely with case managers, behavioral health teams, and other staff to ensure integrated care delivery.

Maintain relationships with local clinics, hospitals, and specialists to streamline referrals and transitions of care.
Participate in daily huddle and weekly IDT team, monthly MDT meetings and contribute to the development of care protocols and policies.
Administrative and Compliance
Ensure compliance with facility policies, healthcare regulations, and confidentiality standards (e.g., HIPAA).
Maintain accurate and up-to-date records in the electronic health record (EHR) system.
Assist with reporting requirements for grants, funding sources, and regulatory agencies.
Participate in setting metrics, goals, and clinical outcomes for recup clients.
QUALIFICATIONS
Education and Experience
Licensed vocational nurse (LVN), registered nurse (RN), or equivalent healthcare background preferred.
Minimum of 2 years of experience in a healthcare or social services setting; experience with vulnerable populations is highly desirable.
Skills and Competencies
Strong organizational and time-management skills.
Excellent communication and interpersonal skills, with cultural competence and sensitivity to diverse populations.
Knowledge of chronic disease management, mental health, and substance use disorders.
Familiarity with community resources and healthcare systems, especially those serving unhoused individuals.
Certifications
Current CPR, BLS, and First Aid certification.
Physical Demands and Work Environment:
Ability to work in a fast-paced environment, handling multiple priorities and adapting to shifting needs.
Occasional travel to attend meetings.
May require occasional evening or weekend hours depending on client needs.
Available for triage 24/7.
