JOB SUMMARY
The Medical Coordinator is a Licensed Vocational Nurse (LVN) and has apivotal role in overseeing medical care for homeless clients, ensuring high standards of service, and compliance with organizational policies. This position requires you to be on call 24 / 7.
KEY RESPONSIBILITIES
o Facilitate hospital discharges and oversee medical treatment plans.
o Create care plans, coordinate support services, make recommendations for care partners, establish weekly goals for patients for medical or social reintegration
o Coordinate medication regimens and follow-up appointments.
o Conduct initial medical assessments and manage emergency situations.
o Maintain client compliance and transport clients as needed.
o Work with other team members to ensure that there are enough medications and other supplies on hand
o Oversee client meals, ensure delivery, coordinate special dietary needs with kitchen staff
o Constructively manage difficult situations and seek help when needed
o Facilitate the hospital discharge process and engage patients with the program
o Communicate effectively with medical staff to ensure high efficacy of care
o Audit charting for medical staff ensuring accurate and detailed documentation for medical billing
REQUIREMENTS & QUALIFICATIONS
Current and non-restricted LVN license in California with 2 years of relevant
experience
Ability to make informed decisions under pressure.
In-depth knowledge of medical terminology and practices.
Strong communication skills and ability to build patient relationships.
Preferred : Home Health Nursing experience and / or experience working with populations
experiencing addiction and behavioral health conditions
Preferred : Bilingual in English and Spanish

Paid time off upon completion of 90-day probationary period
Salary : $70,000 - $75,000
