JOB SUMMARY
The Street Medicine Provider delivers direct medical care to individuals experiencing homelessness in their own environment, like encampments or public spaces, by providing physical exams, medication management, wound care, basic health screenings, and connecting them to necessary community resources like housing and social services, while building trusting relationships to address their unique health needs and advocate for their well-being; essentially, bringing healthcare directly to those who might not access traditional medical settings due to homelessness.
KEY RESPONSIBILITIES
- Direct patient care : Performing physical examinations, taking medical histories, diagnosing illnesses, prescribing medications, and providing basic treatments on-site.
- Outreach and engagement : Actively seeking out individuals experiencing homelessness in their living environments to establish rapport and assess their health needs.
- Case management : Coordinating care with other community agencies, linking patients to housing assistance, food banks, mental health services, and other support systems.
- Health education : Providing information about hygiene, disease prevention, and healthy lifestyle choices tailored to the needs of homeless individuals.
- Documentation and reporting : Maintaining accurate medical records, documenting patient encounters, and submitting reports to relevant organizations.
- Collaboration with team members : Working closely with social workers, peer navigators, and other healthcare providers within a street medicine program.
