POSITION: Nurse Practitioner or Physician Assistant
DESCRIPTION:
WholeLife is seeking a knowledgeable and compassionate Family Medicine Nurse Practitioner (NP) or Physician
Assistant (PA) to join our multidisciplinary team. In this role, you will provide a full spectrum of family medicine
services, delivering personalized care to patients from pediatrics to geriatrics. This role is ideal for a practitioner
who values comprehensive, patient-centered care and is dedicated to building lasting relationships with
patients.
- Comprehensive Patient Care: Provide assessments, diagnoses, and treatment plans across a diverse
patient population, including children, adults, and elderly patients.
- Preventive and Chronic Care Management: Offer preventive care services such as immunizations,
screenings, and health counseling, while managing chronic conditions such as diabetes, hypertension,
and asthma.
- Pediatric and Adolescent Care: Conduct well-child visits, manage common childhood illnesses, and
provide guidance to parents on child health and development.
- Adult and Geriatric Care: Address health concerns specific to adult and aging populations, including
wellness exams, managing complex medical needs, and coordinating with specialists when necessary.
- Collaboration and Coordination: Work closely with our physicians, nurses, and administrative team
to ensure continuity and quality of care.
- Patient Education: Educate patients and their families about health maintenance, preventive
measures, and disease management, supporting informed healthcare decisions.
REQUIREMENTS:
- Graduate of an accredited Advanced Nursing or Physician Assistant Program and hold a current
Texas license
- Minimum of 2 years’ experience in Family Medicine, with specific experience across pediatric, adult,
and geriatric populations preferred.
