Certification Details

ARRT, BCLS certifications required by time of submission.

Job Details

Rotating weekends and holidays included.

Call only on holidays.

Please have all certifications (ARRT, BCLS) by time of submission.

Job Requirements

Primary Source Verification of professional license and / or certification required.

Proof of COVID Vaccine series OR Bivalent booster required.

Proof of 2 immunizations or positive titer for MMR required.

Fit Test for N95 mask is required.

Hepatitis B Vaccination Series or Positive Titer required.

Influenza Vaccine documentation required from September-May.

If history of positive PPD / TB blood test, a Negative Chest X-Ray and Annual TB Questionnaire are required.

Copy Valid Drivers License required.

2 Step PPD OR IGRA (Quantiferon / Tspot) required.

Proof of 2 immunizations or Positive Titer for Varicella required.

Proof of Tdap Immunization within 10 years required.

Additional Details

Proof of COVID Vaccine series OR Bivalent booster required; Medical exemption reviewed and approved prior to start.

Hepatitis B Vaccination Series, Proof of Immunization, Fit Test, and other medical documents must include identifiable information.

Federal, County, National and Statewide Background Check required.
