Social Worker
The Social Worker will coordinate and provide social services for guests of the Courter Memory Center. The services will be provided in accordance with the philosophy, mission, standards, policies, and procedures of the Wilbur and Charlotte Courter Memory Center.
Duties/Responsibilities:
- Develops and maintains the social service program, drafting supporting policies, procedures, and guidelines for the program. Develops and maintains a thorough knowledge of referral resources throughout the service area.
- Assess guests’ psychosocial status as it relates to dementia and Alzheimer’s conditions. Maintains accurate and timely records including clinical and progress notes according to department policy.
- Understands the resources available to families
- Aids in assessing psychosocial status.
- Works closely with members of the care team in the development of the plan of treatment, based on the psychosocial assessment and accepted social work interventions.
- Develops working relationship with leaders in community resource groups, representing the Memory Center at meetings and on various committees that address the social needs of guests.
- Acts as resource person for other members of the care team and family members, providing in-service education and consultation in understanding the social and emotional factors related to health problems.
- Maintains knowledge of the field including Medicare, Medicaid, and other third-party reimbursement sources.
- Participates in planning and implementing quality improvement activities.
- Provides positive customer relations in dealing with guests, families, physicians, fellow employees, other departments, referral resources, service agencies, and others.
Qualifications Candidates must have:
- Bachelor’s degree in either Social Work – Master’s degree preferred.
- Excellent verbal, written interpersonal communication skills.
- Ability to work independently on assigned tasks as well as to accept direction on given assignments.
- Able to work collectively with the administrative team associates.
- Knowledge of MS Office
- Must report, as required by Michigan law, any suspected incidence of adult abuse to Adult Protective Services, and the Executive Director
Required Clearances:
- Successfully pass required background checks
- Complete CPR, First Aid Trained, and complete training in Blood Borne Pathogens

Pay: $28.00 - $32.00 per hour
Expected hours: 30 per week
