Community Habilitation/ Respite Supervisor and Housing Specialist
Salary Range: $20.50 - $25.00/hr depending on relevant experience
This range reflects about 15 years of experience

Education/Experience/Skills:
- High School Diploma or GED
- 3 years experience working with developmentally disabled or related experience and/or training.
- Computer Skills:
- Word processing, E-mail and database software, apple and google suite programs.

Who we are looking for:
The right person to supervise and coordinate staff activities; to directly support individuals with developmental disabilities in home and community; and to work with stakeholders to help individuals maintain housing subsidies and live independently.
Essential Duties and Responsibilities are divided into 3 parts: include the following. (Other duties may be assigned.)
Respite Supervisor (.15)
Assists in coordinating, supervising, developing staff and managing work schedules for Respite Staff.
Participates in the interviewing and hiring of new Direct Support Professionals.
Works collaboratively with individuals & families as appropriate.
Participation in Life Plan meetings and the person-centered planning process when needed.
Maintains a person centered approach and participates in activities to build competencies and integrate individuals into their communities
Completes, monitors and trains staff on all mandated reporting requirements
Group Community Habilitation (.50)
Assists individuals we serve to identify social, recreation and other community resources the individual may wish to access and devise plans to achieve that access
Negotiates schedules and daily activities directly with the individual and/or caregiver & establishes and maintains regular hours with individual(s) that you work with .
Completes all mandated reporting requirements by Wildwood and regulating agencies.
Housing Specialist .(35)
Assists Individuals in maintaining their rental subsidies and in maintaining subsidy compliance & maintains working relationships with individuals receiving housing assistance and their circle of support.
Coordinates tracks and collects necessary paperwork for housing subsidy needs
Maintains working relationships with individuals landlords and assist with mediation of concerns when necessary
Collaborates with business office to maintain accurate records and billing requirements &
Completes monthly notes related to billing needs and requirements and in compliance with OPWDD regulations
Qualifications
Other Requirements:
- A valid NYS driver's license and ability to drive and driving record acceptable for agency insurance coverage are required.
- Daily access to a vehicle.
