Self-Direction Services Coordinator
Salary Range: $24.15-$29.55/hr depending on verifiable experience
Latham office/hybrid WFH after first 60 days

Bachelors Degree in a Human Services-related field preferred.
- This may be substituted with an Associates Degree and one-year of experience in working with people with Intellectual and Developmental Disabilities or QIDP status.

Who we are looking for:
We are looking for a Self Direction Services Coordinator that will be at the main point of contact for all Self-Hired, Self-Direction staff.

Essential Duties and Responsibilities include the following: (Other duties may be assigned)
- Overseeing a caseload of up to 60 individuals. This includes:
- Processing all reimbursements for individuals, vendors, and staff. This is inclusive of gathering all needed documents to ensure that reimbursements meet all Medicaid and OPWDD requirements, and ensuring participants review all staff reimbursements.
- Reviewing all staff notes on a monthly basis to complete monthly narratives for all goal-related services (Com Hab/SEMP)
- Monitoring expenses and reviewing monthly expenditure reports for each individual. Distributing monthly expenditure reports to each individuals team with a summary of usage.
- Acting as the main point of contact for the individuals self-directed services, which involves contact over email, phone, or text with the participant and all members of their team of supports.
- Attending biannual Life Plan meetings for all individuals
- Reviewing Life Plans to ensure all Medicaid and OPWDD requirements for service delivery and billing are met
- Reviewing Staff Action Plans created by Support Brokers to ensure they match the Life Plan and contain appropriate staff actions for person-centered service delivery.
Additional Requirements:
- A valid NYS driver's license, daily access to a vehicle and driving record acceptable to the agency insurance carrier's standards are required.
