Job Title: Homeless Services/Rapid Rehousing Coordinator
Position Type: Full-Time Non-Exempt
Reports To: Associate Director
Job Purpose
Our mission is to assist and enable low and moderate-income individuals to obtain the opportunities needed to prepare themselves for self-sufficiency. The Homeless Services/Rapid Rehousing Coordinator is key in supporting their team members daily. They provide clients with the skills, knowledge, and information to obtain and maintain housing while developing client-centered goals to achieve holistic stability. They must have experience with case management and goal writing, be analytical and self-motivated, confidently exercise independent judgment, and strongly desire to make a difference in low to moderate-income communities.
Duties and responsibilities
· Oversee and conduct TBRA/ESG case management with clients experiencing homelessness
· Completing assessments, scales, intakes, and referrals for customers needing services.
· Oversee and mentor staff with case management of clients participating in agency programs
· Coordinate monthly in-home case management appointments to review income and expenses, create budgets, and create goals to achieve self-sufficiency
· Supportive services for Rapid Rehousing programs include in-home case management, landlord engagement, unit location, inspections, lease review, and mediation of tenant/landlord relationships
· Participation in the COC Coordinated Assessment Process
· Client Intake activities include applying for ID cards, birth certificates, government phones, DHS, and SS benefits
· File Development, maintenance, accounts payable, data entry, reporting, and monitoring grant spending
· Information/outreach/referral
· Other duties as assigned
Qualifications
· 4-year degree preferred
· Required Case Management Experience
· Proficiency in Microsoft Office, including Word, Excel, and PowerPoint
· Experience with HMIS preferred
· Experience with State and Federal Grants
· Proven organizational skills
· Motivated self-starter with excellent oral and written communication skills
· Previous or related experience in community action with people in poverty or homelessness.
· Strong people skills and the ability to exercise sound judgment in complex situations.
· Multi-lingual a plus
· Valid Illinois Driver’s License
· Pass a Criminal Background check
