Job Summary
Provides intake, follow-up linkage case management, and crisis response to clients seeking services or reentering services post-hospitalization. Candidate must be able to travel to other sites as needed.

Qualifications
Associate degree from an accredited institution in Behavioral Science.

Pay: $22.22 - $32.20 per hour
Expected hours: 37.50 per week
