GRANT-FUNDED ROLE: (HRSA Grant #227883 Title, Community Health Worker)
GRANT END DATE: February 2027
WORK SCHEDULE: Monday- Friday, 9a-5p. There may be some early evenings, 11a-7p, and weekend hours , 9a-1p, to be scheduled in advance.
Position Overview:
The Care Technician will provide intensive prevention and support services to Behavioral Health Expansion Program participants; link patients to Behavioral Health and Substance Use Disorder Treatment, care and support services through intensive outreach, escort services, advocacy, and health education, using various models, including screening, brief intervention, and referral (SBIRT) for Mental Health and Substance Use Disorder (SUD), and referral to Ryan’s Medically Assisted Treatment (MAT) program. This work is carried in support of the mission and goals and Ryan Network.
Essential Functions:

Assess patients for support services programs and case management services, through completion of intake/assessment, reassessment process and development of service plan.

Document all patient encounters in the electronic medical record, ensuring that client and patient records are maintained in accordance with agency protocols and standards.

Conduct rapid HIV testing as needed.

Collaborate with community partners and stakeholders to identify individuals appropriate for the Behavioral Health Expansion Program, including referral to MAT program, where indicated.

Educate potential patients about the Behavioral Health Expansion program and encourage their participation in the program.

Facilitate behavioral and adherence counseling for clients receiving Behavioral Health Expansion program services.

Educate community residents and agencies about Ryan Health services and programs.

Where indicated, conduct community outreach on nights and weekends.

Provide home, hospital, or community site visits and escorts, as indicated.

Ensure coordination and intensive follow-up of medical, psychiatric and social services. Schedule and facilitate patients’ attendance at medical and other appointments.

Implement, monitor, reassess and document activities to ensure that patient needs are met.

Practice accepted infection control measures in accordance with OSHA Blood Borne Pathogens Standards, and the Center’s policies and procedures.

Collaborate and consult with team and other service providers in identifying service gaps for case planning activity.

Develop and expand the network of communication and cooperation with other health, mental health, drug treatment, and social service provider agencies working with targeted populations.

Advocate and negotiate services on behalf of patients, family and significant others, in accordance with Center policies and procedures.

Facilitate crisis intervention and referrals to emergency services.

Assist in program compliance with funding requirements, including data collection and timely and accurate submission of information/reports to supervisor.

Implement strategies to reach target populations.

Educate patients regarding early medical intervention, options to reduce/modify stressors, and adherence to treatment regimens, and appointment schedules.

Attend required meetings, supervision, case conferences, conferences, in-service seminars, as assigned.

Immediately report any problems/unusual occurrences to supervisor.

Perform other duties as assigned.
Qualifications
Minimum Experience and Skills Required:
- 1-2 years of social services, health education or care coordination experience.
- Experience providing supportive and/or educational services to MAT patients.
- Experience working with various intervention models.
- Strong communication and interpersonal skills.
- Ability to work flexible hours, including evenings, nights and weekends.
Education, Licenses and/or Certification Required and Skills:
- High school diploma or equivalent
- NYS Driver’s License required
Preferred Qualifications:
- BSW or BA in Health or Human Services
- Bilingual English/Spanish. French Creole
- Medicaid/CHIP/managed care experience
- Certification in EBIs preferred
Minimal Training Time:
90 day probationary period
