Key Responsibilities

- Co-facilitate assigned sessions and assist in modifying problem behaviors.
- Perform initial and follow-up assessments for patients, developing individualized treatment plans as needed.
- Ensure accurate and timely documentation of patient care, treatment, and incidents.
- Act as a liaison between referral sources and patients, providing crisis intervention and aftercare planning as necessary.
- Communicate with insurance carriers and coordinate case management duties.
- Evaluate patient needs and determine necessary referrals to other programs or facilities.
Qualifications and Requirements
- Bachelor's or Master's degree in a social or health services field preferred.
- Experience in counseling, instruction, internship, or similar practicum through education or certification process required.
- Licensure or certification for counseling, as required by the state where the facility operates.
- CPR and de-escalation/restraint certification required.
- First aid may be required based on state or facility regulations.
