What are the responsibilities and job description for the Patient Flow Coordinator position at Willis Knighton Health?
This position is for a day shift Patient Flow Coordinator.
Duties:
· Collaborates with entity Administration, Nursing Supervisors and Managers, Case Management, the Business Office, Patient Access, Environmental Services, Doctor’s Offices, and all admit sources to ensure proper placement of each patient
· Maintains expertise in all associated software and communication tools for the assignment of beds and the communication of pertinent information, such as account number, room number, ETA, etc., to the sending/receiving areas
· Performs the Pre-Registration of any incoming patients, the transfers of patients from department to department, and ensures that all needed prior authorizations are obtained prior to placement
· In conjunction with the RN staff, facilitates both in-house patient transfers to different levels of care and transfers coming from other facilities, and coordinates the transport, keeps report, and notifies the sending facility/location of each placement
· Facilitates the verification of insurance, authorizations, point of service amounts due, and any other required documentation in a timely manner
· In conjunction with the RN Staff, ensures that state and federal requirements and joint commission standards are met with regard to transfers and level of care requests and reports all concerns
Experience & Certification:
Minimum Education Required: High School graduate or equivalent (Some college or bachelor's degree preferred)
Experience in Customer Service, Patient Access, Transport, Nursing Tech, or a department that deals specifically with patient flow and multidepartment communication/coordination preferred.
Licensure/Certification Required: CHAA or LPN (*CHAA certification required within 6 months of being hired)
Skills Required:
- Computer keyboarding/typing and multi-line telephone skills
- Basic knowledge of Microsoft Word, Excel, PowerPoint, and Outlook
- The ability to read and decipher data in order to find anomalies or issue areas, convert any given data into percentages and averages, and to quickly create custom graphs or tables from reported data upon request.
- Excellent interpersonal skills, phone answering skills, excellent verbal communication skills with good dictation and proper grammar, and has a basic understanding of medical terminology
- Ability to work in a multi-tasking, fast paced environment with an electronic bedboard application under minimal supervision and being able to stay productively occupied during low volume periods
- Ability to embrace the mission and vision of Willis Knighton and its effort to creating the utmost in patient flow efficiency
