We are currently seeking a Social Service Director to lead our social services department and ensure the well-being of our residents and their families.
Job Responsibilities :
- Oversee the planning, development, and implementation of social service programs for residents.
- Conduct psychosocial assessments and develop care plans tailored to residents’ needs.
- Provide counseling and support to residents and their families regarding emotional, social, and financial concerns.
- Assist with discharge planning, including coordination with community resources and services.
- Advocate for residents’ rights and ensure compliance with state and federal regulations.
- Work collaboratively with the interdisciplinary team, including nursing, therapy, and administrative staff.
- Maintain accurate and timely documentation of social services provided.
