**Job Summary:**
The Certified Home Health Aide (CHHA) at Willow Spring Hospice Care provides compassionate, hands-on care to patients in their homes or in hospice facilities. Under the supervision of a Registered Nurse (RN) or Licensed Vocational Nurse (LVN), the CHHA assists with personal care, activities of daily living, and basic health-related tasks to ensure the comfort and dignity of patients receiving end-of-life care.
**Key Responsibilities:**
- **Personal Care:**
Assist patients with activities of daily living (ADLs) such as bathing, grooming, dressing, toileting, and feeding. Ensure patient comfort and safety while maintaining their dignity and respect.
- **Basic Health Support:**
Monitor and record vital signs, report any changes in the patient’s condition to the RN or LVN, and assist with simple wound care and other basic healthcare tasks as directed.
- **Emotional Support:**
Provide companionship and emotional support to patients and their families, offering comfort during difficult times. Help create a calming environment that supports the patient’s well-being. - **Mobility Assistance:**
Assist patients with mobility, including transfers from bed to chair, walking, and repositioning to prevent pressure sores.
- **Light Housekeeping:**
Perform light housekeeping duties related to patient care, such as changing bed linens, laundry, and tidying up the patient’s living space.
- **Documentation:**
Maintain accurate and timely documentation of care provided, including ADLs, vital signs, and any changes in the patient’s condition.
- **Team Collaboration:**
Work closely with the hospice care team, including RNs, LVNs, social workers, and chaplains, to ensure comprehensive care that meets the patient’s needs.
- **Compliance and Safety:**
Follow all hospice care protocols, safety guidelines, and regulations to ensure the highest standards of patient care.
**Qualifications:**
- **Certification:**
Current certification as a Home Health Aide (CHHA) in the state of California.
- **Experience:**
Previous experience in hospice, home health, or a related healthcare setting is preferred.
- **Skills:**
Strong interpersonal and communication skills. Compassionate, patient-centered approach to care. Ability to work independently and as part of a team.
- **Other Requirements:**
Valid driver’s license and reliable transportation. Ability to travel to patients’ homes and other care settings within the service area.

Pay: $35.00 - $40.00 per hour
