Overview
The Medical Social Worker at Willow Spring Hospice Care plays a vital role in supporting patients and families through the emotional, psychological, and social challenges of end-of-life care. This position involves assessing the psychosocial needs of patients, developing care plans, providing counseling, and connecting families with community resources. The Medical Social Worker works as part of an interdisciplinary hospice team to ensure holistic, compassionate care.
Duties
-Psychosocial Assessment:
Conduct comprehensive assessments of patients' and families' emotional, social, and financial needs. Identify strengths, support systems, and areas of concern that impact the patient's end-of-life experience.
- Care Planning:
Collaborate with the interdisciplinary team, including physicians, nurses, chaplains, and other healthcare professionals, to develop and implement individualized care plans that address the psychosocial needs of patients and families.
- Counseling and Support:
Provide individual and family counseling to help patients and their loved ones cope with the emotional and psychological aspects of terminal illness. Offer grief support, crisis intervention, and guidance throughout the hospice care process.
- Resource Coordination:
Identify and connect patients and families with appropriate community resources, including financial assistance, support groups, legal aid, and other social services. Assist with advance care planning and end-of-life decision-making.
- Advocacy:
Advocate for the rights and needs of patients and families within the healthcare system. Ensure that patients' wishes and preferences are respected and honored in their care plans.
- Bereavement Support:
Participate in the development and implementation of bereavement programs and services, providing follow-up support to families after the death of a loved one.
- Documentation:
Maintain accurate and timely documentation of assessments, interventions, and patient progress in compliance with hospice regulations and company policies.
- Team Collaboration:
Participate in interdisciplinary team meetings, case conferences, and care planning discussions. Contribute to the overall coordination and delivery of hospice care.
Qualifications:
Master’s degree in social work (MSW) from an accredited institution.
Minimum of 2 years of experience in medical social work, preferably in hospice, palliative care, or healthcare settings.
- Licensure/Certification:
Licensed Clinical Social Worker (LCSW) in the state of California is required. Certification in Hospice and Palliative Care Social Work (CHP-SW) is a plus.
- Skills:
Strong assessment, counseling, and communication skills. Ability to work independently and as part of an interdisciplinary team. Compassionate and empathetic approach to patient care.
- Other Requirements:
Valid driver’s license and reliable transportation. Ability to travel within the service area to meet with patients and families in various settings.
Working Conditions:
The role involves working in various settings, including patient homes, hospice facilities, and office environments.
- Physical Demands:
Requires the ability to travel to patients' locations and perform psychosocial assessments. Occasional lifting or movement of patient-related materials may be required.
Please note that this is a per diem job and is paid as $70-$75 per visit.
