Job Description
We are seeking a highly skilled Registered Nurse to serve as our Medicare MDS Coordinator at Willow Tree Care Center in Delta, Colorado. As a key member of our team, you will play a vital role in ensuring the highest quality care for our residents.
Responsibilities:
- Maintain current knowledge of applicable managed care, Medicare, and state Medicaid regulations.
- Accurately complete all resident assessments and appropriate care plans to meet resident care needs.
- Schedule residents for admission, quarterly, annual or change of condition resident assessment and oversee communication to nursing and other departments.
- Monitor MDS and care plan documentation on all residents, assuring that complete, organized and chronological documentation is on file in the resident chart.
