About the Role
We are looking for an experienced Registered Nurse to join our team as a Skilled Nursing Community Health RN. In this role, you will be responsible for coordinating the implementation of the Medicare/PPS program guidelines and monitoring documentation to ensure compliance with program guidelines.
Key Responsibilities:
- Maintain current knowledge of federal/state regulations regarding the MDS and care planning process.
- Demonstrate ability to accurately complete all resident assessments and appropriate care plans to meet resident care needs.
- Assist with pre-admission screenings to determine/prepare for resident care needs.
- Coordinate and facilitate interdisciplinary team meetings to gather information/data, communicate changes and maintain/update records.
