Job Description
We are seeking a highly skilled Registered Nurse to join our team at Willow Tree Care Center in Delta, Colorado. As an RN Healthcare Specialist, you will play a vital role in coordinating the facility's Medicare Program and Resident Assessment Instrument (MDS) processes.
The ideal candidate will have experience working in an acute or long-term care setting and be proficient in completing resident assessments and care plans. You will also assist with pre-admission screenings, maintain current MDS status, and schedule residents for assessments.
You will be responsible for ensuring that all resident care meets or exceeds facility guidelines and complies with state and federal regulations. This includes monitoring documentation, facilitating interdisciplinary team meetings, and maintaining accurate records.
Essential Functions and Responsibilities:
- Maintains current knowledge of applicable managed care, Medicare, and state Medicaid regulations.
- Maintains current knowledge of federal/state regulations regarding the MDS and care planning process.
- Demonstrates ability to accurately complete all resident assessments and appropriate care plans to meet resident care needs.
- Assists with pre-admission screenings to determine/prepare for resident care needs.
- Maintains current MDS status of every resident according to state/federal guidelines.
- Schedules residents for appropriate admission, quarterly, annual or change of condition resident assessment and oversee communication to nursing and other departments.
