Job Description:
In this role, you will be responsible for coordinating the facility's Medicare program, overseeing resident assessments, and maintaining accurate and up-to-date documentation. You will work closely with our interdisciplinary team to ensure that our residents receive high-quality care and services.
Responsibilities:
- Coordinates the overall coordination of the facility Medicare Program, Resident Assessment Instrument (MDS) and care planning processes.
- Monitors MDS and care plan documentation on all residents, assuring that complete, organized and chronological documentation is on file in the resident chart.
- Coordinates and facilitates interdisciplinary team meetings to gather information/data, communicate changes and maintain/update records.
