What are the responsibilities and job description for the Hospice CNA position at Willowbrook Hospice?
Willowbrook Hospice is a regional hospice located in Georgia– we are not a corporation. We have been serving Atlanta & North Georgia for over 5 years, and we are growing to serve more communities throughout North Georgia with our new location in Winder, GA. We operate as a family with each individual team member and the patients we serve.

Description:
The CNA works under the direct supervision of the RN Case Manager and/or Patient Care Coordinator to:
• Assist with personal care of patient, i.e., bathing, feeding, bed linen care, mouth care, vital signs, transferring and shaving.
• Document in patient’s chart via EMR (HCHB)
• Follow infection control and safety measures.
• Assist with patient/family teaching concerning patient care and care for the caregiver.
• Bereavement activities as requested and/or able.
• Clean and monitor supplies.
• Attend and participate in staff and patient review meetings when able.
• Maintain and uphold the Hospice Code of Ethics.
• Keep Patient Care Coordinator/RN Case Manager informed of condition of patient/family.
• Maintain appropriate number of supplies at patient’s residence.
• Provide emotional support to patient/families.
Minimum Requirements:
• Current CNA License in the State of Georgia
• Current CPR & First Aid Certification
• 1-2 years hospice experience (other candidates may be considered with experience in assisted living, home health, long term care)
• Compassion & Integrity
• Driver’s license, insurance and vehicle
• Excellent Time Management Skills
