Position Description
Responsible for delivering rehabilitative and therapeutic services ordered and supervised by licensed medical staff and in accordance with accepted standards of practice, state and federal regulations and licensing requirements. Assists residents to achieve or maintain their highest practicable level of ADL function through consistent, structured and controlled re-teaching and retraining techniques to promote increased participation in activities of daily living. Principal Responsibilities
Provides restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc. within the scope of the training; Utilizes restorative techniques including verbal cueing, task sequencing, hand over hand, prompting, guarding, etc.; Promotes greatest degree of independence possible for residents.
- Assists residents with their physical needs including eating, bowel, and bladder training, and assistance following incontinence but encourages residents to perform these functions themselves to their capacity or ability; Participates actively in retraining programs for these functions.
- Observes and reports on resident’s conditions and documents observations as required. Contributes knowledge of the resident’s conditions / needs to resident care plans; Documents restorative exercises and weekly summaries as required.
- Assist in the care and maintenance of therapy area and equipment. Assures that equipment is clean and functional.
- Assists with lifting, positioning and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., in keeping with specific resident safety needs; Assists residents in preparation for and transporting to appointments, activities and social programs
- Participate in meal service including serving, clearing, and cleaning the dining room; Assures that residents are positioned appropriately for meals; Assures that appropriate utensils are within reach of residents; Serves food trays and assists with feeding as indicated; Delivers and documents all a.m., p.m., and bedtime snacks and supplements; Provides hydration to all residents according to center policy.
- Checks residents in section regularly; Answers call light promptly; Follows through with resident problems or needs by delivering care or reporting to supervisor depending upon level of ability and nature of resident need.
- Maintains resident stability by checking vital signs and weight; testing urine; recording intake and output information.
- Provides resident comfort by utilizing resources and materials; transporting residents; answering resident call lights and requests; reporting observations of the resident to nursing supervisor.
- Documents actions by completing forms, reports, logs, and records.
- Protects company’s integrity by keeping resident information confidential.
- Receives assignment from supervisor and performs duties in accordance with each resident’s detailed plan of care, providing care to residents who are presently receiving restorative ADL nursing interventions; Reviews care plans routinely to determine if changes in the residents’ care routines have been made.
- Participates in and receives nursing report upon reporting for duty and during shift change.
- Receives training from therapists and supervisors on the restorative techniques and performs techniques.
- Maintains knowledge of resident’s condition at times. Notes significant changes in the condition of residents and takes appropriate action; Identifies emergency situations and responds with appropriate measures.
- Takes and records temperature, pulse, respiration, intake and output; Reports resident incidents to immediate supervisor; Communicates requests and problems to appropriate personnel; Takes BPs upon request by Supervisor.
- Observes safety needs of residents including use of bedrails, rail pads, and other protective supports when ordered; Identifies safety and security hazards and reports to appropriate personnel.
- Assists resident with personal care, including nails, hair, oral hygiene, and dressing following Standard Precautions Encourages resident to do as much as he or she can do him or herself; Cares for resident’s prosthesis as required; Maintains resident privacy when providing care.
- Prepares residents for bed rest; Gives special care to residents in casts or traction such as monitoring comfort, skin color, and temperature. Moisturizes resident’s skin; Applies deodorant.
- Follows policies and procedures related to body mechanics, transfer, body positioning, and mobility as outlined in the nursing policies and procedures manual; Repositions residents as needed using supportive devices when necessary (handrails, pillows, etc.) and reports the same.
- Reports and documents self-care ability, assistance needed and; resident’s progress or lack of progress. Assists in resident’s transition from prescribed O.T., P.T., S.T., to restorative program and from restorative program to maintenance by CNA.
