Responsibilities

- Provide ostomy care and assist with wound management.
- Conduct assessments and document patient health information accurately.
- Assist in case management by coordinating patient care plans with healthcare teams.
- Support geriatric patients by addressing their unique health needs and concerns.
- Implement pain management strategies as directed by supervising healthcare professionals.
- Educate patients and families on health coaching techniques for better health outcomes.
- Lift and move patients safely while adhering to proper lifting techniques and safety protocols.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Maintain compliance with all healthcare regulations and standards.
Skills
- Proficient in medication administration and understanding of pharmacology.
- Strong knowledge of basic life support (BLS) techniques and emergency response protocols.
- Experience with durable medical equipment (DME) is a plus.
