EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

  • Associates degree in nursing or human services field required.
  • Bachelor's degree in social work or human services field preferred.
  • One or more years’ experience in a clinical setting preferred.
  • One or more years’ experience in discharge planning preferred.
  • Crisis Prevention and CPR certifications (offered on site)
  • Ability to work with male and female adult (18 ) patients with chemical dependency and mental health disorders
  • Teamwork and excellent customer service

ESSENTIAL FUNCTIONS:

  • Based on patient's needs, develop a continuing plan that promotes their long term health and relapse prevention.
  • Responsible for setting discharge appointments, including group or individual therapy, psychologist or psychiatrist.
  • Make arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care.
  • Ensure all documentation regarding patient discharge planning is clear, concise and timely.
