What are the responsibilities and job description for the Discharge Planner position at Wilmington Treatment Center?
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
- Associates degree in nursing or human services field required.
- Bachelor's degree in social work or human services field preferred.
- One or more years’ experience in a clinical setting preferred.
- One or more years’ experience in discharge planning preferred.
- Crisis Prevention and CPR certifications (offered on site)
- Ability to work with male and female adult (18 ) patients with chemical dependency and mental health disorders
- Teamwork and excellent customer service
ESSENTIAL FUNCTIONS:
- Based on patient's needs, develop a continuing plan that promotes their long term health and relapse prevention.
- Responsible for setting discharge appointments, including group or individual therapy, psychologist or psychiatrist.
- Make arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care.
- Ensure all documentation regarding patient discharge planning is clear, concise and timely.
