Responsibilities of MDS/Care Plan Coordinator:
*Actively participate in the development of a comprehensive, individualized plan of care for each resident.
*Ensure resident plan of care is completed, updated, and reviewed in accordance with the RAI guidelines and changes in resident care/needs.
* Assume responsibility for implementation of Clinical and Prospective Payment System Minimum Data Set completions schedule
* Direct, organize and schedule required resident assessments and related documentation for Medicare, Medicaid, managed care and Health Maintenance Organization (HMO) reimbursement
* Coordinate and oversee all functions of the MDS assessment process to assure compliance with federal and state requirements
* Participate in pre-admission process to determine potential residents' level of care
* Participate in resident level of care determination upon admission
* Participate in tracking changes on residents' level of care to assure timely scheduling of MDA assessments
* Review all MDS assessments and related forms to certify timely completion by signing the MDS forms on the appropriate sections
* Review reports of timely completion of residents' MDS assessments and care plans and take follow up action as necessary
* Analyze Quality indicator reports for potential resident concerns and problems and review residents' care plans to be sure this information is included
*Will maintain a current knowledge base regarding state and federal regulations relating to the RAI process, care planning and the prospective payment system (PPS).
Qualifications of MDS Coordinator:
· RN/LPN License
· MDS Experience
