Description
Summary: The LVN provides skilled nursing care to patients in the home adhering to the plan of treatment written by the RN. Certain clinical training and experiences will be required of the LVN.
Essential Duties and Responsibilities: include the following. Other duties may be assigned.
- Under the direction of the RN, provides nursing care in accordance with departmental and hospital policies and procedures.
- Involves the patient, family and/or significant other in all phases of care.
- Demonstrates substantial knowledge of medications and their uses.
- Demonstrates substantial knowledge related to the care of geriatric patients.
- Communicates information related to patient progress, needs and/or condition changes to physicians, agency staff, patients, family, and/or others as applicable.
- Organizes work so that priorities are well defined and completes and submits documentation in a timely manner.
- Promotes and maintains an agency environment that is in compliance with federal, state, and Local regulatory agencies.
- Promotes a positive work environment and demonstrates cooperation with other agency staff and patients.
- Dresses in a professional manner, following dress code.
- Demonstrates knowledge of teaching, leadership, and management principles.
- Maintains confidentiality of patient information and respect for patient behavioral response to illness.
- Works as scheduled and follows attendance policies: Reports to duty on time; reports illness in advance of shift; accepts assignments and completes them using good time management; makes every effort to be on duty as scheduled; will be available to work every other weekend, if scheduled; will be flexible with regard to number of patient visits seen in a day; will be available to accept new assignments on any scheduled day and respond to new assignments within one hour of notification.
- Participates in in-services and continuing education programs and attends all agency staff meetings.
- Participates in mandatory hospital educational activities, which include, but not limited to: Fire, Disaster, and Hazardous Materials.
- Maintain CPR certification
- Maintains all patient records up to date no later than 5 business days.
Qualifications
Education and/or Experience
LVN in the State of Texas; Graduate of an accredited school of nursing; CPR. Home Health experience preferred.
Other Skills and Abilities
- Computer literacy and adaptability required
- Maintain own internet accessible device with reliable internet access
- Maintain own cell phone with reliable network coverage
- Excellent organizational, communication, and judgment skills are a must.
- Interpersonal skills necessary to communicate effectively with various levels of hospital staff.
- IV therapy, Wound Vac Therapy, and venipuncture experience preferred.
- Reliable transportation with insurance coverage, Texas Driver’s license; CPR
