Description
The Case Manager, BSW integrates the duties as case manager by performing discharge planning evaluation of assigned patients. The Case Manager, BSW is responsible for care coordination and development of the discharge plan through collaboration with the healthcare team and internal / external resources. BSW functions as a primary contact for coordination of internal and external resource services to facilitate care delivery in the discharge plan. As a member of the healthcare team they communicate effectively with the interdisciplinary team to ensure safe, quality care is provided to all patients.
