Job Summary
We are seeking a compassionate and dedicated Direct Support Professional to provide care and support to individuals with disabilities. The ideal candidate will assist in daily activities, offer emotional support, and ensure the well-being of our clients.
Duties
- Collaborate with the DSP Supervisor and the individual to tailor and implement personalized activity plans to meet the diverse needs of each participant.
- Provide adept hands-on assistance for daily living activities and personal care, ensuring individual comfort and well-being.
- Facilitate skills development through creative and targeted training programs, adapting strategies to individual learning styles.
- Plan and participate in community outings, showcasing the ability to foster social inclusion and adapt to diverse environments.
- Collaborate effectively with a multidisciplinary team, demonstrating strong interpersonal and Communication skills.
- Conduct regular activity assessments, demonstrating analytical skills to track individual progress and adjust support strategies accordingly.
- Maintain accurate and detailed documentation of participant records and progress reports, demonstrating organizational proficiency.
- Ensure a safe and supportive environment, adhering to all relevant regulations and guidelines.
- Communicate proactively with individuals, their families, and team members, exhibiting strong communication skills. FOR NIGHT ONLY: Check the resident’s periodically throughout the night and attend to any matter that needs attention. Check every 30 minutes at minimum and log.
- Be responsible for necessary office work i.e., filing, phone calls, and use of office equipment.
- Be available to attend any crises that arise. (Another staff member will be always on call in the event of an emergency).
- Log any pertinent or required information before leaving your work shift.
- Inform incoming staff of any developments requiring their attention.
- Oversee physical care of each resident.
- Assist with development of acceptable habits and attitudes in residents.
- Assist with management of resident behavior.
- Help to meet the goals and objectives of any required service plan.
- Assist in developing treatment plans for all residents of the house, whenever possible.
- Attend all staff meetings and special meetings as required by the Program Director.
Abilities
- Demonstrate knowledge of direct support services.
- Must have a strategic mindset and ability to self-direct.
- Must have the ability to multitask and take direction from management.
- Problem-solving skills independently and in a group setting.
- Must show written and verbal communication skills.

Qualifications
- Familiarity with working with people with disabilities such as intellectual and developmental disabilities
- Knowledge of home health practices and patient care
- Strong communication skills and empathy
- Ability to work collaboratively in a team environment
- Patience, compassion, and a genuine desire to help others
Education and Experience
- A high school diploma or GED with experience working with ID/DD & Mental Health
adults/children
- Must be 18 years of age or older to provide services to residents
All Training Requirements must be fulfilled within (90 days) of hire

- CPR/1st aid Certificate
- Current DMV Driving Record and Copy of Driver's License
- Copy of Social Security Card
- High School or College Transcripts (highest level completed)
- Central Registry Background Check
