Certification Details BLS VA License Job Details Position Template Facility Name : Winchester Medical Center Unit : Home Health Position Information : Role : Physical Therapist Scope of Role : Home health PT – will provide direct patient care in the patient's home environment as part of a multidisciplinary team. The therapist will evaluate patients and use professional judgment to design the individualized plan of care which best meets the patient's needs. The therapist will use guided interventions to improve the safety and functional independence of its clients through the rehab process by providing direct, one-on-one patient care, and will also supervise care provided by assistants. Must have at least 1-year Home Health experience, ability to use laptop for documentation and cell phone for communication. Patient Ratio : 5-7 points per day (number of patients could vary based on visit type such as Start of Care vs. Routine Visit) Weekly Guaranteed Hours : 36 Shift Hours : Mon-Fri 8a – 4 : 30p (5x8’s) Weekend Requirements : As needed, but typically every 5th Weekend On-Call Requirements : None Duration of Assignment : 13-26 weeks Years of Experience : 1 year Job Requirements Required Certifications / License : BLS and VA License (please include VA License, if they don’t have include status of license) and Driver’s License 1 year Home Health experience Schedule Information Shift Hours : Mon-Fri 8a – 4 : 30p (5x8’s) Weekend Requirements : As needed, but typically every 5th Weekend On-Call Requirements : None Unit Specific Information Unit : Home Health Number of Beds : N / A Patient Ratios : N / A Equipment : N / A EMR : EPIC
