Job Description
The Senior Clinical Manager will oversee the clinical performance of teams within Assisted Living and Memory Care, providing care at the highest practicable level using the Erickson Person-Center Approach model.
This role offers a unique opportunity to lead the clinical aspects of a new Assisted Living building, working closely with the Assistant Administrator, ADON, and Assisted Care Manager.
We are seeking an experienced RN with a minimum of 5 years of experience in Home Health, Assisted Living, and/or Memory Care settings. The ideal candidate will have management experience and be comfortable working with electronic medical records (EMRs).
Responsibilities include:
- Coordinating and providing clinical oversight and care to residents
- Partnership with Assisted Living Manager for resident pre-screening, admission, re-admission, and ongoing assessments
- Ensuring individualized service plans reflect current needs and resident preferences
- Tracking and trending clinical quality areas, including wound care, infection control, falls risk reduction, pain management, weight loss, and dehydration
