Key Responsibilities:
- Provide clinical oversight and care coordination for residents in Assisted Living and Memory Care
- Collaborate with the Assisted Living Manager on resident admissions, assessments, and ongoing care planning
- Ensure compliance with state, local, and company policies and regulations
The successful candidate will possess a current RN license, a minimum of 5 years of experience in a Home Health, Assisted Living, or Memory Care setting, and excellent leadership and communication skills.
