Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will be responsible for providing high-quality patient care. The LPN will work closely with registered nurses and physicians to ensure that patients receive the best possible care during their treatment journey.

Duties

• Administer medications and treatments as prescribed by physicians.
• Conduct diagnostic evaluations and assist in discharge planning for patients.
• Participate in memory care programs, supporting patients with cognitive impairments.
• Perform specimen collection processing and phlebotomy as required.
• Utilize electronic health record systems such as Point Click Care for documentation and patient management.
• Collaborate with healthcare team members to develop and implement individualized patient care plans.
• Wound Care

Experience

• Previous experience in Long term care is preferred.
• Familiarity with diagnostic evaluation processes and discharge planning is highly desirable.
• Experience with specimen collection processing and phlebotomy techniques is a plus.
• A strong commitment to providing compassionate patient care while maintaining professionalism at all times.

Pay: $22.50 - $24.50 per hour

Expected hours: 40 per week
