This Memory Care Director position is responsible for assisting the Wellness Director and Executive Director in all areas necessary to promote optimal resident care and efficiencies of the Memory Care/Memory Care Unit including being the Director of the Activity Department. This position coordinates all activities for resident engagement, as well as coordinate the care services provided to residents by caregivers and medication aids in collaboration with the Wellness Director and Executive Director including care plan meetings. This is a working coordinator position thus responsibilities also include providing personal assistance and routine daily care in accordance with the Resident Care Profile to help promote the residents’ independence and quality of life. This position is also responsible for coordinating recreational activities and encouraging residents to maintain optimal functionality. The position may also have a responsibility for staff development within the Memory Care/Memory Care Unit. This position is for Memory Care/Memory Care units with thirty (30) or less residents.

DUTIES AND RESPONSIBILITIES:

1. Primary responsibility is to assist with a creative social atmosphere throughout the property and promote resident and family participation through regularly scheduled events, activities, special programs and guest speakers, movie showings, exercise sessions.
2. Assist with the coordination and attendance of special functions and activity programs for major holidays.
3 Ensure activities and events acknowledge and meet the cultural, religious and ethnic diversity and interests of the residents and their families.
4. Follow standards set forth by the move-in evaluation/lease information. Ensure provision of services for the residents and ensure that all Federal and State policies governing resident care are closely monitored and carried out.
5. Assist residents with their Activities of Daily Living (ADL) tasks as needed and directed. This includes bathing, grooming, personal and dental hygiene, transportation to meals and activities, bowel and bladder functions (e.g., take to bathroom, portable commode, change incontinency products, etc.), shaving, dressing, and hair care.
6. Respond to resident’s needs promptly while maintaining each resident’s self-respect, personally dignity, personal safety, and confidentiality.
7. Oversees continuing education and staff development programs for caregivers and medication aides, as regulated by Federal and State licensing requirements.
8. Conduct or participate in orientation for new members of the department to ensure that all employees are knowledgeable of the goals, policies and procedures for the department.
9. As directed by the Wellness Director and Executive Director, monitor and assist with all personal care services and medication provision provided to community residents. Ensure that all appropriate documentation has been completed, including charting, ADL documentation and medication documentation.
10. At the direction of the Wellness Director and Executive Director, assist with the completion of resident assessment in accordance with current rules, guidelines and company policies that govern resident assessment; administer pre-admission and periodic assessments to all residents and monitor changes in status. Provide reassessments on a regular basis, as requested by the Wellness Director and Executive Director.
11. As directed by the Wellness Director and Executive Director, complete all required data entry into the Care software to ensure maintenance of resident care information.
12. Ensure that Care Profile/Individualized Service Plan and assignment sheets are completed timely, and all required documentation has been completed.
13. Medication management and resident care as required.
EDUCATION AND EXPERIENCE:
LPN preferred, but not required. Prefer at least two-years of college coursework. Prefer one (1) year experience directing an activity program in a long-term care setting.
Mathematical Skills: Ability to perform simple mathematical calculations for preparing reports.
Reasoning Skills: Understand complex issues and ability to carry out plan of action on own initiative. Oral/Written Communication Skills: Demonstrates sound communications skills, both verbally and
written in dealing with residents, staff, families, visitors and physicians. Must be able to speak, read and write English.
Equipment Used: Be proficient in basic office equipment (including a personal computer, desktop applications, mobile applications, mobile (smart) devices, and phone systems) for the purpose of accomplishing and maintaining a high level of job performance.
Physical Effort: Be mobile and able to perform physical requirements of the job. Will require physically assisting residents during activities.
Personal Characteristics: Displays sincere compassion towards cognitively impaired older adults. Demonstrates genuine concern for the physical and emotional needs of older people and their families.
Working Conditions: Well-lighted, safe office with appropriate ventilation and temperature levels. Will be working in all resident care areas of the facility. Will accompany residents on outings outside of the property. Minimum Age: Due to auto carrier’s restrictions, must be at least twenty-one (21) unless not going to drive for the facility, then minimum age is (18)
