Job Title : Substance Use Navigator
Salary : $2912 per week
Contract Duration : 13 weeks

Location : Lakeport CA
Job Role : Assisting emergency room and inpatient identified patients with substance and alcohol treatment.
Responsibilities include identifying resources working with community partners connecting patients with outpatient mental health services and intervening in suspected abuse / neglect cases.
Additional responsibilities include identifying and addressing social determinants of health providing cross coverage for inpatient and birthing center and participating in committee review hearings.
Requirements : MSW degree (LCSW not required). Minimum of 1 year of ED experience. SUN and Case Management experience preferred.
Working Hours : 5 x 8 MFwith flexibility to start ator 0930 for an 8 hr day.
Additional conditions : No weekend or oncall requirements.
