DUTIES AND RESPONSIBILITIES:

Assist professional nursing staff by performing duties in caring for residents at facility; work under supervision of professional nurses; transport, bathe, and dress residents; comb hair, clean teeth, and care of nails; toilet residents, make beds and feed residents; execute resident assessment and care plans; provide for safety, comfort, and wellbeing of resident. Execute proficient computerized documentation.

QUALIFICATIONS:

LICENSE: License/Certification/Registration are required by federal and state law.

CONTINUING EDUCATION: As required by federal and state law, including in-service training programs.

EDUCATION: Minimum of a high school diploma and completion of the State approved training program, including on-the-job training.

EXPERIENCE: Prefer some previous experience in health care facilities. Knowledge of procedures and techniques involved in administering simple treatments and providing related bedside resident care service. Understanding of standard techniques used in providing personal services for resident and in caring for equipment and supplies. Good understanding of basis of asepsis and sterile techniques to avoid infection of residents and self or contamination of equipment and supplies. Be aware of the assessment process, and the development and execution of care plans.

JOB KNOWLEDGE:

STANDARDS.  Perform duties in accordance with established methods and techniques and in conformance with recognized standards. Provide maximum resident care services within limits defined by delegated tasks.

OTHER.   Exhibit genuine interest in residents; patience and tact in dealing with residents, families, and visitors; initiative and judgment in recognizing symptoms indicative of resident' s adverse reactions to treatment; willingness to perform a variety of simple repetitive tasks.

FUNCTIONS:

  1. Ambulate and transfer residents utilizing appropriate assistive devices and body mechanics.
  2. Routinely tum and position residents. If on bed rest turn every 2 hours, if skin issues turn every 2 hours.
  3. Make beds every day, changing linens per facility policy and as needed.
  4. Measure and record residents' heights and weights.
  5. Take temperature as directed by the nurse.
  6. Care for incontinent residents.
  7. Measure and record residents' intake and output.
  8. Assist residents in all activities of daily living.
  9. Pass out trays and feed/assist residents as needed.
  10. Observe and document food, nourishment, and fluid intake of each resident.
  11. Keep always assigned resident bedside area clean and personal belongings in orderly arrangements.
  12. Assist with development and review of the resident care plans in conjunction with nursing and other disciplines.
  13. Ensure resident privacy, respect, and dignity.
  14. Assist in orientation of new nursing assistants.
  15. Participate in in-service programs.
  16. Exhibit and perform proper hand washing techniques.
  17. Maintain proper infection control procedures when handling clothing and linens.
  18. Receive oral reports and written assignments from charge nurse at beginning of each shift.
  19. Provide maximum resident care services to assure well-being of resident to greatest degree.
  20. Execute duties and responsibilities in conformance with established routine.
  21. Assist with execution of resident assessment and plan of care.
  22. Assure physical comfort, safety, and mental well-being of residents.
  23. Assist in daily requirements and tasks m care and treatment of residents, such as toileting, grooming, bathing, feeding, dressing, observing intake of food, care of hair and nails, moving residents from area to area, responding to calls, etc.
  24. Needs to work well with other departments
  25. Detect and correct situations that have a probability of causing accidents or injuries to residents.
  26. Observe and report symptoms, reactions, and change of residents.
  27. Understands and follows appropriate procedures and techniques.
  28. Understands and follows the resident's individualized care plan.
  29. Understands standard techniques in personal service-care of residents.
  30. Be aware of sterile procedures and avoid contamination.
  31. Immediately answer the call light of any resident.
  32. Encourage and motivate residents to participate in recreational, occupational, and physical therapies and other rehabilitative measures to promote activity of mind and body.
  33. Foster self-respect and esteem by recognizing, accepting, and respecting those entrusted to his/her care.
  34. Clean used equipment and return to proper place after using, such as wash basins, bed pans, urinals, and any other equipment that may be used.
  35. Record bowel movements and urine output of all residents and/or as directed by charge nurse.
  36. Check residents frequently, and before going off duty, to be sure that they are clean and dry.
  37. Sign equipment in and out.
  38. All charting must be entered into computer before end of shift — Green — Chart on it.
